This article [J. Biomed. Opt. 17, 117005 (2012)] was originally published online on 7 November 2012 with an error on p. 2. The first matrix in the left column

{A} = ‖ \begin{matrix} \cos θ & \sin θ \\ - \sin θ & \cos θ \end{matrix} ‖,

has been replaced by

{Q} = ‖ \begin{array}{l} [\sin^{2} ρ + \cos^{2} ρ \exp (- i δ)] & [\sin ρ \cos ρ (1 - \exp (- i δ))] \\ [\sin ρ \cos ρ (1 - \exp (- i δ))] & [\cos^{2} ρ + \sin^{2} ρ \exp (- i δ)] \end{array} ‖ .

This article was corrected online on 21 November 2012. It appears correctly in print.
